A Health & Safety Commission report into the management of contractors on the railways says this week that duty holders must accept responsibility for the safety of operations. It also emphasised the need to develop and retain a well trained and skilled workforce, and that established principles for contractor management must be followed.

It says that recent rail crashes show that there have been significant contractor management failures and that new initiatives must be reflected in better practice on the ground.
