in brief Crew on the sand dredger Sand Kite

Crew on the sand dredger Sand Kite failed to maintain an effective lookout before it collided with the Thames Barrier in 1997, according to the Marine Accident Investigation Board (NCE 30 October 1997). MAIB also discovered the vessel had no voyage plan and that crews lacked vital skills. The collision caused £2M of damage to the barrier.
